How to UnBan website from Google search?

If some website get banned by Google and doesn't appear in Google search at all, How is it possible, and what procedure needs to be done in order to recover website and get crawled by Google again in order to appear in Google search?

Your suggestions and recommendations are welcome as i need to do it for one very important website for my friend. Website is well optimized, but recently banned for unknown reason.

If the site has received a Manual Action from Google then it is necessary to fix the problems identified and then submit a reconsideration request.

Thank you very much for your guides. It really helped a lot. Yeah, website was not in Google webmaster tools, and wen i submitted, shortly after i have seen reason of removal. I cleaned website for all bad things i could see and think of, then i hit request review, so hopefully it will fix the problem.

Gee, I didn’t know that there is a reconsideration request for Google to remove the ban on a site. But maybe it will depend on the type of case for banning the said site. For honest mistake like keyword stuffing maybe it will take a short time for Google to unlock the site for eligibility to Google Search. But for other grave cases maybe the ban will stay.
