If I find an image that is offensive or copyright infringement on a site, how can I ask Google to remove it from their search index?
I think you don't need to remove the image from google image result, because it will help you to increase the visitor that came from image searching. But you may need to baypass the google image frame, so that, the visitor that came from google image search will redirect to your page site links, immediately.

Just visit on google webmaster tools, remove your link on it.
Webmaster tools are best for it.
Demote the image links in Google webmaster site.
Also don't forgot to do the same with Yahoo/Bing Webmaster tools.
If you have a good reason like infringement of copyright then maybe it is best to report it to Google. But that image will still appear until Google makes its move. With the offensive image it would depend on the viewpoint because it may be offensive to you but it may not be offensive to me. But on second thought why would Google remove an image on a site? Isn’t that intrusion of property?
