Add CSS to a long form and some PHP for the mailer

Hi

I need a pretty simple job doing, will take a little time tho. ... So current job is this..

I have an application form, little bit long as a HTML form. So the HTML coding already done for the main design. There are about 35+ fields. I will send you the basic template.

What I need is this adjusted with 3 different themes using CSS. I need the CSS clearly labelled so that each part is clearly described. It doesn't need to be special, just clean professional looking and each of the 3 need to look a little different. There are basic things in there which need validation. Must input to most fields, email, tel number etc, simple stuff. Also a text box probably needed at the bottom with terms and conditions in it, and a tick box after that to confirm client happy with terms.

Second part is also pretty simple. I need the code and installation instructions for a php mailer to work on the cpanel. It needs to have basic protection / security. Obviously this is to forward the form contents to an email. If the form does not validate then message needed to indicate where content needs adding or correcting. And a success notification if good.

This form/mailer will be an add on to an existing wordpress site. The HTML form will be installed as a sub domain and needs appropriate php mailer installed on the server.
