We need a feedback form, which will be hide like the image given below.
We need the script inside a div, so that it can be used in any web pages.
When people will click on a the button like this it will open the feedback form.
Practically we need it exactly like the given image. Better visit the mentioned site and study it for some time.

i'm going to try to provide this in a div, but i'm not sure if i can since form action="php file" requires a external file. i will try other things and maybe just pasting the whole php file into it but i can't promise anything. i will use a frame, like in flipkart.com. i'm also going to use some css animations.
