How to remove web spam from Google

Should you report a page that is web spam? How can this be done? Also, what would you consider to be a spammy page?

If the page is clearly misleading you would want to report it, especially if it were ranking for keywords on Google search and the content is about something else. That could be a subtle form of web spam.
