I want my site to have a form for user to fill in after the video finished playing in this theme of my site here: http://melaniemar.com/video/
This page use build in video background from the theme and it's auto play. You must add the hidden form and show up after the video stop playing or finished playing.
You could do whatever methods as long as the final result is like what I want.
