For this gig, I will design from scratch (not just simply convert your form unless you wish to do so) a simple form in Adobe LiveCycle or Adobe Acrobat which can include your logo and up to 10 fields. For every additional set of up to 10 fields, please add an extra gig. Fields can include text fields, check-boxes, radio buttons, text boxes, buttons (including email submit button).
You can use your form with the free Adobe Acrobat Reader.
